Patient Resources

Ask Me 3
for patients - This is the Ask Me 3 page for patients. It has a lot of
information for patients about improving your health literacy.
2.
Medication Card - This card was developed by the Rhode Island
Health Literacy Project. It can help you keep track of your medications when
you go to the doctor.

Family
Medical Tree - This resource is from the Rhode Island Health Literacy
Project, just click on the link that follows to view a Spanish or English
version on the family medical tree. Listing your family medical history before
your visit, makes your appointment less stressful and you do not have to
remember so many things.

Island Health Literacy Project and you can use it as a list
to remember everything you need for your medical appointment.
