Birth and Death Certificates
Cost is $15.00 per certified copy of a birth certificate.
Cost is $13.00 for the first copy of a death certificate and an additional charge of $10.00 for each additional copy of a death certificate purchased at the same time.
To receive a copy of a birth or death certificate, please complete the appropriate form below and either mail it to us or bring it in to one of our offices. Please note that these are applications only.
The Jefferson County Health Department charges an additional $6.00 for birth and death certificates that are sent by certified mail.
Birth and death certificates sent by regular mail are sent at no charge for postage.
Effective July 14, 2010 the health department will assess a $1.00 (one dollar) check fee for all services paid for by check.
EFFECTIVE APRIL 1, 2011 ALL MAIL-IN REQUESTS MUST BE NOTARIZED.
The Credit Card by Mail form is a printable form. Please download this form and complete it if you wish to pay by credit card through the mail. Submit it with the Birth or Death certificate appliacation.
