Community Health

The Community Health team strives to promote the health and safety of Jefferson County residents through education, community partnerships, policy change, advocacy, and resources.  We provide many services including educational presentations, wellness screenings, nutrition consults, child care clock hour training, car seat checks and distribution, cooking classes, and more.  We also work with School Wellness Program Grant, Adult Brain Injury, and Children and Youth with Special Health Care Needs.

Health Education Classes

Health Education provides various presentations for all ages on topics such as nutrition and physical activity, healthy habits, and tobacco and substance abuse. To schedule a class or learn more, contact us at 636-797-3737 ext. 139

Car Seat Program

Health Education has 2 car seat technicians who provide car seat safety checks and car seat distribution.  If you are interested in a car seat safety check or in need of a car seat, call for more information or to set up an appointment: 636-797-3737 ext. 127 or ext. 106.

Adult Brain Injury

The Adult Brain Injury (ABI) Program is a grant funded program through the Department of Health and Senior Services. The ABI Program provides assistance in locating, coordinating, and purchasing rehabilitation and psychological services for individuals who have survived a traumatic brain injury (TBI). The ABI Program provides two primary services: service coordination and rehabilitation services.

Children and Youth with Special Health Care Needs

The Children and Youth with Special Health Care Needs (CYSHCN) Program isa grant funded program through the Department of Health and Human Services.  CYSHCN provides assistance statewide to individuals from birth to age 21 who have or are at increased risk for a medical condition that may hinder normal physical growth and development and who require more medical services than children and youth generally.  CYSHCN provides two primary services: service coordination and limited funding for services for medically necessary diagnostic and treatment services.
