Cost is $15.00 per certified copy of a birth certificate and $13.00 for the first copy of a death certificate. An additional charge of $10.00 for each additional copy of a death certificate purchased at the same time.

To receive a copy of a birth or death certificate, please complete the appropriate form by below and either mail it to us or bring it in to one of our offices. Please note that these are applications only.

Effective 5/1/2007, the Jefferson County Health Department charges an additional $5.21 for birth and death certificates that are requested to be mailed.
