VITAL RECORDS
Three easy ways to obtain Birth or Death Records
Choose the method that best suits your needs:
Walk-in
Birth and death certificates are available at the Health Dept from 8:00 a.m. - 5:00 p.m., Monday thru Thursday. Same-day service is available to walk-in customers. You must have a photo identification card of some kind, e.g. Driver's license, work I.D., etc. to obtain the copies.

FEES:
Birth Certificates - $15.00 each
Death Certificates - $13.00 for 1 copy
$10.00 for each additional copy per person
You may complete the attached application and send it to us with payment. For mail-in requests you must have the form notarized. If you want the certificate sent back to you by certified mail there is an additional $6.00 fee.
The Credit Card by Mail form is a printable form. Please download this form and complete it if you wish to pay by credit card through the mail. Submit it with the Birth or Death certificate application.

You may order a certified copy of a birth or death certificate online using your credit card through our only approved internet ordering service, VitalChek Network Inc. An additional fee is charged by VitalChek for using this service, and all major credit cards are accepted.
Other internet vendors are not authorized by Jefferson County Health Department and may charge additional fees without providing state-approved services. Neither Jefferson County Health Department nor VitalChek have control over the services and fees other internet vendors offer and charge, nor can we guarantee the confidentiality of any vital record data provided to vendors other than VitalChek.
